​Cheeseburners FHC Code of Conduct, & Declaration.

Well, we may be Cheeseburners but we've got to abide by them there rules!  Make sure you read them thoroughly so that you can understand club expectations. A 'declaration' shall be request at the start of each club ran event.
​​All PLAYERS, PARENTS/GUARDIANS and OTHER FAMILIY MEMBERS MUST AGREE to the following club CODE OF CONDUCT prior to registration for each season/camp:
Each participant and their parents/guardians are required to acknowledge the Code of Conduct before each season/camp begins and must abide by the Code of Conduct throughout the entire program.  Violations of the Code of Conduct may result in disciplinary action including suspension - either temporarily or permanently as determined by the Club Director.

Code of Conduct for Players
  • I will be a good sport to players, coaches, officials, opponents, and parents/guardians at every game and practice, no matter win or lose.
  • I will learn the value of commitment by participating in as many practices and games as I can.
  • I will encourage my teammates and praise good efforts.
  • I will be honest, fair, and respectful to others at all times.
  • I will aim to learn all I can from sports.
  • I will arrive at practice on time and be ready to dedicate my attention to my coaches.

Code of Conduct for Parents/Guardians
Parents/Guardians and other family members are welcome to watch practices/games from the sidelines/bleachers. We require that a distance be kept between you and all players/practices for your own safety and to ensure players can focus on the task ahead.
  • I will ensure that I and other family members/guests put the emotional and physical well-being of youth participants ahead of a personal desire to win.
  • I will ensure that I and other family members/guests respect players, coaches, officials, spectators, and families at all times.
  • I will ensure that I and other family members/guests respect the decisions of officials or coaches.
  • I will ensure that I and other family members/guests do not engage in any violence or verbal threats or use any profanity.
  • I will ensure that I and other family members/guests will not yell advice to my child during a game.
  • I will ensure that I and other family members/guests model good sportsmanship for all youth participants.
  • I will ensure that I and other family members/guests do not belittle or ridicule anyone involved in a youth sporting event–in public, private, in-person, or online.
  • I will ensure that I and other family members/guests do our part to keep sports fun and positive for every youth participant.
DECLARATION OF GOOD PHYSICAL CONDITION & CONSENT TO EMERGENCY MEDICAL TREATMENT
I declare and acknowledge that (or on behalf of my minor child), to the best of my knowledge, I (or minor child) am in good physical condition and have no disease or injury that would impair my performance or result in my being injured during any Cheeseburners Field Hockey program participation.  I hereby consent (or on behalf of my minor child) to emergency medical treatment, hospitalization or other medical treatment as may be necessary for the welfare of myself (or minor child) by a Certified Athletic Trainer, Physician, or other appropriate healthcare professional, and/or hospital in the event of an injury or illness during the periods of time in which they are participating in a Cheeseburners Field Hockey program.  I understand and agree that all related costs are my responsibility.  I acknowledge and agree to: the risk of bodily injury, including without limitation scrapes, broken bones, paralysis, dismemberment, and death; and knowingly and freely assume all such risk. This includes injury of damage sustained while and/or resulting from use of any equipment or training provided by Cheeseburners Field Hockey.   I (or on behalf of my minor child) promise not to sue Cheeseburners Field Hockey, the coaches, staff, board members or volunteers of said organization with respect to any and all such assumptions of risk.  I acknowledge that I have carefully read this ASSUMPTION OF RISK, WAIVER AND RELEASE OF LIABILITY agreement, fully understand its terms, understand that I have given up substantial rights by signing it, including the right to bring a legal action or assert a claim against Cheeseburners Field Hockey or the coaches, staff, board members or volunteers of said organization, and sign it freely and voluntarily. 
​​COVID-19 - All PLAYERS, PARENTS/GUARDIANS and FAMILY MEMBERS MUST take ownership and monitor their own health situations.  Those who have come into contact with someone with COVID-19 or have been identified to self monitor, PLEASE DO NO ATTEND any club events until you have been given the all clear.  All coaches, players, parents/guardians and other family members attending Cheeseburners Field Hockey events or representing the club at events:
  • must adhere to the local COVID-19 (school/host/event) protocols for the wearing of masks

Please note that we shall continue to adjust these rules as necessary.  Cheeseburners Field Hockey and its operatives has the right, at any time, to make adjustments and decisions to best benefit the group as a whole based on local and federal guidelines and new information.
