Cheeseburners FHC FALL '23 Outdoor Program[s].

NEW FOR FALL 2023!  Cheeseburners FHC is happy to offer two different sign-up opportunities for TRAVEL and/or ACADEMY players.  Please note that at the time of checkout you will be asked if you require a team jersey(s) - if needed, these are an additional $30 each.

1.  TRAVEL - FULL FALL SEASON - Open to players (U10-16) who plan on competing on behalf of the Cheeseburners for the fall season.  This includes practices, games, and tournaments.  Please note that travel for the fall season shall be limited to Illinois (up to 1.5 hours one way).

TRAVEL 'FULL' FALL SEASON 2023 - $350
  • Up to 17 x 1.5 hour practice sessions
  • Games scheduled for U14 Pacers and U12 Rangers/U10 Super 10s levels (and in some cases our U8 Sportsters)
  • Two Tournaments (Narwhal Games - WI 09/30, Pumpkin Classic - IL 10/22)
  • Training w/Concordia University (at Tomasini Stadium) 09/12
  • Concordia University vs. Centre College (SAA Champions) 09/16
  • Concordia University vs. Marian University (Battle of Wisconsin) 10/28
  • End of Season Banquet 10/28
  • Access to Hocktoberfest w/Milwaukee Admrials
  • Sibling rate available! (50% second household child)

2.  ACADEMY - UP TO FIVE SUNDAY SESSIONS - For U8-U14 players.  Continued development for U8s, and U10 players who cannot commit to 'Travel' requirements.  Also, for those Middle School players looking for additional training sessions.  Players may be offered an opportunity to participate in a scheduled Cheeseburners game. 

ACADEMY 'PART' FALL SEASON 2023  - $110 
  • Up to 5 x 1.5 hour practice sessions
  • Games may be scheduled for U10 Super 10s and U8 Sportsters
  • Concordia University vs. Centre College (SAA Champions)
  • ​Concordia University vs. Marian University (Battle of Wisconsin)​ ​
  • Access to Hocktoberfest w/Milwaukee Admirals

EXPECTATIONS:
  • Commitment Levels - FULL FALL SEASON players are expected to attend the vast majority of practices and games - there MAY be some exceptions, but this should be considered the rule.  
  • Duration - Program shall commence August 13 and concludes October 28.
  • Experience Required - Unless otherwise contacted, you must be an incoming 4th thru' 8th grader (Fall '22) to join our team. No prior field hockey experience is necessary.
  • Home Field - Southeast Sales Powersports Stadium @ Uihlein Soccer Park
  • Competition - Local and regional programs include Arrowhead Youth U10-U14), Brookfield Academy (U14), Lift Field Hockey (U10-U14), University Lake School (U14), University School of Milwaukee (U12-U14), and Windy City Field Hockey (U10-U14)

*DECLARATION OF GOOD PHYSICAL CONDITION & CONSENT TO EMERGENCY MEDICAL TREATMENT
I declare and acknowledge that (or on behalf of my minor child), to the best of my knowledge, I (or minor child) am in good physical condition and have no disease or injury that would impair my performance or result in my being injured during any Cheeseburners Field Hockey program participation.  I hereby consent (or on behalf of my minor child) to emergency medical treatment, hospitalization or other medical treatment as may be necessary for the welfare of myself (or minor child) by a Certified Athletic Trainer, Physician, or other appropriate healthcare professional, and/or hospital in the event of an injury or illness during the periods of time in which they are participating in a Cheeseburners Field Hockey program.  I understand and agree that all related costs are my responsibility.  I acknowledge and agree to: the risk of bodily injury, including without limitation scrapes, broken bones, paralysis, dismemberment, and death; and knowingly and freely assume all such risk. This includes injury of damage sustained while and/or resulting from use of any equipment or training provided by Cheeseburners Field Hockey.   I (or on behalf of my minor child) promise not to sue Cheeseburners Field Hockey, the coaches, staff, board members or volunteers of said organization with respect to any and all such assumptions of risk.  I acknowledge that I have carefully read this ASSUMPTION OF RISK, WAIVER AND RELEASE OF LIABILITY agreement, fully understand its terms, understand that I have given up substantial rights by signing it, including the right to bring a legal action or assert a claim against Cheeseburners Field Hockey or the coaches, staff, board members or volunteers of said organization, and sign it freely and voluntarily. 
​​*​​All PLAYERS, PARENTS/GUARDIANS and OTHER FAMILIY MEMBERS MUST AGREE to the following club Rules and Regulations prior to registration for each season/camp:
Parents/Guardians and other family members are welcome to watch practices/games from the sidelines/bleachers. We require that a distance be kept between you and all players/practices for your own safety and to ensure players can focus on the task ahead.​  It is the responsibility of the parent(s)/guardian(s) to their players and other family members prior to a practice/game about being mindful of all players’ and coaches' personal space while in their presence. You must respect each individual and his/her comfort levels with social distancing and physical contact.

COVID-19 - All PLAYERS, PARENTS/GUARDIANS and FAMILY MEMBERS MUST take ownership and monitor their own health situations.  Those who have come into contact with someone with COVID-19 or have been identified to self monitor, PLEASE DO NO ATTEND any club events until you have been given the all clear.  All coaches, players, parents/guardians and other family members attending Cheeseburners Field Hockey events or representing the club at events:
  • must adhere to the local COVID-19 (school/host/event) protocols for the wearing of masks

Please note that we shall continue to adjust these rules as necessary.  Cheeseburners Field Hockey and its operatives has the right, at any time, to make adjustments and decisions to best benefit the group as a whole based on local and federal guidelines and new information.
